- 5 – 7 Passport size colour photos.
- Admit card of class 12th.
- Mark sheet of class 12th or equivalent.
- Certificate of class 10th against proof of age.
- UPCATET hall ticket.
- Transfer Certificate / Migration Certificate / School Leaving Certificate.
- Caste certificate.
- Call letter and rank card.
- Affidavit by the student & parent.
